Registration Office Forms

Honouliuli Middle School Enrollment
For all new students enrolling at Honouliuli Middle School the following forms can be completed prior to coming into the Registration Office.  Forms are also available at the Registration Office at the time of enrollment.  Please complete and print out the following form(s):
 
  • HMS Student Enrollment Form
  • Verification of Residence HMS
  • Special Services Questionnaire
  • MV - 1 Form
  • Student Publication Audio/Visual Form
  • Tech Responsible Use Form
 
Student Information Update
If looking to update student information please complete and submit the Student Information Update Form.  If updating your physical or mailing address please submit the Student Information Update Form with proof of residence (Mortgage, Lease Agreement, or Utility Bill).
 
Notary Form
If you are residing with another family, please complete the Notary Form for Residence.  Submit the Notary Form with the following documentation:
1.  Mortgage or Rental Agreement of the family you are residing with
2.  Utility Bill of the family you are residing with
3.  Documentation that you are residing at the address
 
Home Schooling
If your child will be homeschooled, complete and submit the Exceptions to Compulsory Form (4140) to the Registration Office.
